yes, but rarely, sometimes it might not work
i herad some times it kills few people...i think it is usles. Can win battle with that, or make big casualties. thaks for resp
It has never worked for me, but my theory is that its supposed to kill people if you don't attack the city. Maybe like starving them out in Warband?
I read on taleworld forum that for some people it kills a few soliders (17-20). But it is to risky to escape after being discovered. So I thik in my game i will skip this. Destroing part of the sity walls is the one for me ;)

It kills about 2-8% a day or so of the army, lost the link from my source, but it was on the taleworlds website somewhere, I would still skip this step, as others mentioned, it is VERY risky to escape, I have lost the siege many times purely out of being discovered, I found it to be much easier for the attack if this is done, but more than likely, you'll be discovered, and it leads to no good. Just blowing down the walls & attacking is what I do.

Everyone is Wrong here... Poisoning the Water Supply is the best mechanic a sieger can use...
and no it doesnt use your engineer skill you need your tactics skill to be high, i have tactics at 10 and i do it atleast 50 times without fail, an army of 500 turns to 65 in a matter of minutes without the fighting, also it isnt once per day, you can do it over and over again as long as you dont get caught i usually stop at 50. :) hope that helped
