Level 320 (+140), I think. Tough to get the 140, I haven't got one, much easier to get in the ~135 area.

Where to get it, at the bottom of the Abyss.

I'm sure the main map is pretty much capped at Lvl 320 (+50) (on Way of the Nioh) and you'd probably have to Soul Match to get it to +50, it's been a while, my memory is a bit hazy.

To get the highest level weapons, you have to complete the final floors of the abyss (995-999).

I got my first level 320 weapon on floor 36.

A +137 item is not that hard to get, but what I can say for sure is that something above that is really, really rare.

The highest I got by completing floors 995-999 several times was +137. Then I started to do 995-998, and I got my first +138. 👍🏽
