Cannot find grave after returning to death site

So each time I've died, I've returned directly to my grave site/dead body -no dying again on the way, etc.- but it's no where to be found. The circle in the top left of the hud, with the animal spirit guardian has been empty for the last couple of hours. Even when I did die again, my body/grave is no where to be found, and even after praying at a shrine, the animal guardian does not show up.
I've not modded anything, nor changed settings since my last play session. This whole process was working before, but this time nothing.

Any ideas?

Have you tried calling back the spirit at the shrine?

When your guardian spirit is not in your gauge it means it's at your grave. If you haven't died on the way to your corpse you should be able to get back both amrita and the guardian spirit.Keep in mind if you dropped of a ledge your grave is more likely to be where you fell from than where you hit the ground. If you don't care about the amrita just call him/her back using the shrine "call back" tab.

(if you use a summoner candle you can call the spirit back with all the amrita you had)

If none work then something is bugged, try veryfying files and/or reinstalling.

Hitting X/square (or whatever light attack is) in the guardian spirit selection menu lets you unequip your guardian spirit. That spot being empty results in no living weapon and no graves since those are tied to the spirit. You could check if you accidentally unequiped your spirit at a shrine.
