tab: swap items
spacebar: run/dodge
r: stance modificator and ky pulse (also +left click changes weapon, +alt change ranged weapon, +t change guardian spirit) don't use for actual change stances as you can bind them directly into ta key press.
Mousewheel up: high stance
Centerclick: medium/neutral stance
Mousewheel down: low stance
Everything else is just the same I think.
Regarding the multiple entries, thay are alternative buttons if you wish to have multiple buttons assigned to the same action but they all have the same function.

I binded my keys differently than what you wrote, but it works for me.
However, do you have any idea what's the default key for [RB] on controllers?
Like, on controllers it's RB+B to sheate weapon, but if you hold B it's also used for a weapon skill on some weapons. Now, I have no idea how it is for K+M.
Like, in this case, RB is supposed to be Space on keyboard, and B is supposed to be E.
But when I try this (Space + E) it doesn't seem to work.
I literally tried all combinations of keys + Space, doesn't seem to cut it.
Well, in case you haven't figured it out yet, it's Space+Q with the default bindings; it doesn't take the normal buttons into consideration, there's a specific "Stance" tab in the key binding options.

It think it already does.
However, if they are bound to a different key.
Like... e.g. I currently have mine bound 4 to MW-Down and 5 to MW-Up ('cause my scroller sometimes doesn't respond well xD). So, when I try to change weapons, I click 4/5 and it works.
You have pretty much everything configurable. Split into different sets (that can use same keys), combination and direct shortcuts (stances/swaps), selectable modifier keys, multiple choices on some of the functions, and so on.
Spend some hours adjusting all this stuff to what is comfortable and logical to you (and to what your mouse/its software can do).
FWIW - any self respecting gaming mouse can bind at least any key/button to those - so it can be trivially workarounded.
