UPDATE: The non-starting issue is fixed by port-forwarding the following ports in your router and allowing them in your firewall:
UDP: 3074
TCP: 777, 51967, 3074

This is going to sound so simple it's stupid...
everyone having this problem I am going to guess are using the xbox pc controlers?
Step 1) ok.... plug in controler.
Step 2) Start game
Step 3) goto Options | Control Layouts - switch to keyboard and mouse. Apply settings
Step 4) Start mission / new game
Step 5) once loaded into the level go back to options and change back to your controler and enjoy playing

The port forwarding/firewall changes worked for me and it has been working ever since. :P
Works like a champ. Wish there was a complete workaround without having to use uplay servers for single player though.

This got the game working for me, DX9. DX11 does not load.
This plus setting joystick to default settings (and then changing keys to your preference) worked. If DX11 does not load and you use MSI Afterburner do a profile in RTSS for low detection and viewport.
