I was basically doing a full run last time, forgot to talk to saxophone lady once, did all other stuff, finished game, it didnt trigger. I reloaded at saxophone part, re-did it, got 'Seriously'. But I figured I skipped the tooth too, so I reloaded at library, read ALL the articles, re-did musicians (again), and after leaving and reloading game I finally got 'Make it last'.
I don't know why it didn't register before, as I DID do the tooth quest previously.
Just try and do a FULL playthrough. Id suggest making a copy of your save after each day, just in case you forget something (like I did).
Oh Ive beaten Demontower too, with good ending (needed it for "Seriously?")
I guess it has some backwards-written trigger, or we can't just figure it out.
Good luck with "Seriously?" now
EDIT: Oh wait, 'hold onto anything' not 'make it last'. Durr.
That's strange, still - youre supposed to unlock that at the end of your second playthrough (from what I know. perhaps after meeting some extra conditions)
And oh, there are two .dat files, I think player.dat and persist.dat - player is your current game state, persist is stuff you did in your current and previous playthrough (tho player could hold current playthrough and persistent only general choices, not sure).
Anyhow, go look in c/users/(your nick)/AppData/...roaming? or roaming low? something like that, then /infinite fall (or the other way around)./nitw - and there should be the files.
