how would a monster in attack mode effect people being attacked? there is still a monster pertecting the user.
but if your defensive monster was in attack mode with say 1000 attack points and the enemy has 2000 attack you will suffer 1000 life point damage plus the loss of your monster where as if it was in defence mode with say 1000 defense rating and the monster with 2000 attacked it you will lose the monster but wont suffer life point damage.
as others have pointed out you have been playing the game wrong or simply did not watch the tv show since they played how the cards were played in the game.
of course there are probably magic cards/trap cards and certain monsters with abilites that can bypass/negate some of what i wrote above.

also even though the first series of the show had no direct life point attacks, the toon monsters that Pegasus would use actually have the effect to attack life points directly on cards today let's just say the show is scripted and not 100% card game rules as they have altered based on show and satisfaction of specifics
--in this game you can only attack the players life points if they have no monsters on the field, or stated by specific effects from traps/magic/special effect monster cards, otherwise only a card in attack mode would use subtraction between the two and defense position monsters will take 0 damage towards the players life points unless said effects from another card is stated
