Secured Pharmaceuticals No Keypad Bug
Is this a bug or is it intended? Because if it's a bug, I've been loading and reloading the game for many times, but it never loads the keypad. It was there right at the beginning, but it has gone as I've progressed through the game.

I wish they could fix such nuisances, because this is one of the best games I've ever played. I love everything about it, but such bugs are annoying.

By keypad, do you mean the numeric pad that you can hack or are you saying that there is no key card reader? If the former, it never existted and if the latter, it is bugged.

Yep, the numeric keypad outside the door. I am certain it was there. I even found a keycard for it. Either some quests interfered with each other and caused this or the game is still buggy. I will try to play the game again, and check it to be sure...

You have key code or key card? Key codes are 4-digit codes you get from emails and such. Key cards are physical keys with no numeric codes (thus, no keypad).

To be clear, there are two different types of door locks in the game. One is a numeric keypad that accepts a four digit code and also has a provision for hacking. The second is a similarly shaped key card reader that can only respond to the correct key card. Secured Pharmacuticals has always been secured by a key card reader with no provision for hacking or numeric codes.

There are two key cards for that room available in game and both are found in Crew's Quarters. One is on Regina, a crew member that has morphed into a phantom. If you kill her and don't immediately loot the key, it will be lost forever after the remains despawn. The second key is in DeVries cabin hidden under a lamp.

If you have one of these two keys and there is no circular key card reader on the door to Secured Pharmacuticals, then your game is bugged.

Warc1 exactly. I have the keycard, and the door has no circular keycard reader and it's annoying as hell. I hope they would fix this. Kirk Remmer and Luther Glass side missions are bugged too. Typical Bethesda I guess...

Is that an out-of-power symbol on the keypad? Can you take a closer look and/or give us a closer screenshot? Are the other equipments in the Trauma Center (terminals, dispensers, etc.) out of power as well?

Nope nothing. Everything works in trauma center except this door. It is not related to a power outage or whatever. Frustrating stuff really...

Go up to that black circle where the keypad normally would be, then activate it. You hope that only the "Unlock" image is missing and not the function to unlock it.

Drop the key card, and pick it up again, hoping that maybe that would trigger something. I don't really know.

You hope it's some graphic glitch that you can overcome, and not some nasty bug baked into your saved game. What is the last saved game that didn't have this glitch? Do other doors with this kind of keypad have missing "Unlock" function too?

As a quick aside, I don't think the "Where is Kirk" sidequest is bugged. It has been confirmed that Kirk Remmer does not always spawn when first visiting the Life Support escape pods. If this happens, you can revisit the escape pods after you have restarted the power plant and Remmer will be there.
