Report a translation problem
Here's a couple questions that can help narrow down exactly what is going on:
-Did you load up a savegame from before the end and finish it again? I hear this is necessary to unlock NG+. If this step is skipped, perhaps it improperly gives a NG+ option without resetting the world?
-Did you check the safe/locker in your office to see if any of your equipment from the last run was still there?
New game+ was something I did right after I beat the game, but I'll see if I can find a save that is not too old and see if I can beat it again and create another NG+ save.
The locker had my psychoscope inside of it, but there were no new items in the game. Anything that I looted before hand is completely gone.
It took me a year or more before I got the game, and I never ran into that problem. I figured it was gone. But that might explain why your level data seemed not to be reset. I wonder what steps it takes for the game to half-assedly reset level data on a new game like that...

It seems he thinks it is related to a game or even computer crash related to use of steam cloud. Changes you make to levels is stored in a ".level" file in your savegame folder. He thinks steam cloud incorrectly restores the level data in your save file after you restart. Since the new game essentially deletes the save data for those new levels, steam itself should update its cloud to delete the files too. But a crash skips the steam cloud sync, and steam then helpfully redownloads the missing level data. The game, on your next run, looks and finds already created .level data files, and now the world itself is empty.
I've just encountered this bug for the first time in 480 hours. This resolved my issue, but I didn't toy with cloud save options. I just deleted the files in the Saved Games folder that was under "Campaign0" and when I loaded the game again Steam asked me which files to use (cloud or local) and I chose local. Then I started a new game. I haven't had time to play since I have to work now. Will update as soon as I see no Apex Typhon on the first Lobby map.
EDIT: Confirmed it works as of 5/5/2019.
Had to MANUALLY deleted all saves, REFUSE steam cloud saves, start a new game, finish it, and then start a new game+.
Its a bug that migrates when you have an original save transferred to the newer version of the game when they added NG+.
1. Open Prey (campaign mode)
2. Delete all save files manually within the program's ingame menu.
3. Alt-Tab to your desktop and open into the following directory:
C:\Users\YOURNAME\Saved Games\Arkane Studios\Prey\SaveGames
4. Delete all folders with the denotation of Campaign (it will be listed as Campaign0, Campaign1, and Campaign2).
5. Alt-Tab back into the game.
6. Exit.
Steam Cloud will now update and no longer overwrite a prior cloud save over your game, since it will update based on the folder and savegame edits you made. No Cloud settings will need to be edited in this method. This will need to be done each and every time you decide to play the game fresh, to avoid dealing with the Apex Typhon on your first visit to the Talos I Lobby (as entertaining as that sounds, it's not. It's buggy as hell).
I've just tested this over the course of the week, and it works.
I was never able to pass the "get to main lobby" objective even though I had access to all of the main lobby with cards and codes. It had been so long since I last played, I didn't realize it until I saw somebody in the office who shouldn't have been there.
So I just ran into this problem and followed this solution but I made one small tweak. Delete every thing except the file "NewGamePlus.arkend" in the campaign folder of the campaign that you want to start a new game plus from. This file contains only the data from the campaign that is needed to start a NG+ from. I just tested it and confirmed no apex blob in lobby. Which is great because I prolly would have been done if I had to start completely from scratch. Cheers hope this works for anyone else that runs into the issue.
