I also did not kill him because i wanted the " killed no human" achievement at the end and killing the cook counts for that.
There are actually some side quests you should not complete because they may give you a bad end, like the one from December about fleeing the station with the escape pod.
Also i wonder why you have the "kill cook" quest already. Normally you get that AFTER the events in the kitchen and what you discover there, not before.
Edit: Also, the 1.3 patch just hit if you opt into the Beta branch for the game.
Maybe that fixes that bug.

what do u mean with freezers? no i never had acsses to the kitchen, i gave him the price, and nothing happen, he close the window shutter and thats all.
You disocer the cook, he tells you do find the medal in the crew quarters, after that he tells you to come inside via the side door at the next room, then you need to repair a water valve for him in the kitchen. After that he opens the freezer and tells you to loot it. After the events in the freezer and after what you discover there ( spoiler) the cook has left the kitchen. You then can follow what is told to you by the audio log you find in the freezer.
Which leads you to a meeting with a certain person who then gives you the quest to kill the cook.
So i dont quite get how you have the " kill cook" quest if you never entered the kitchen/freezer.
I think the "kill the cook quest" is not bound on complete the "help the cook" quest.
I found the person who give me "kill the cook" quest per coincidence, i love exploring. i never had a audio log to guide me.
That is strange because that encounter usually does not trigger before you completed the freezer and listened to the audio recording. Banging on the glass does nothing beforehand.
We need a fix! I'm on game+ and I am not going to go back over 4 hours just for this quest, though sad and I really wanted to. It's just too much for a 2nd play through to achieve this again. I just won't like the ending as much (hope that's the only repercussion) .
It's so frustrating to see so many others still having this issue and nothing has been done about it but they still sell the game "as is"?? It's so disappointing because I like this game sooo much.
The only ideas (which I'm sure probably don't apply to you (OP) any longer but may to someone is remote manipulation before the shutter goes down, if possible. If triggered by your phantom, try killing it? Apparently the best course of action is to restart your last save prior to arriving in the kitchen to see if you can do it different. Reason I say this is I had a body disappear yesterday on me that was holding a very necessary keycard after lockdown (you'll know if you've been here, trying not to spoil too much). I had to reload because once she disappeared I couldn't access the necessary area to get out. Honestly, that time I got kinda ticked off. I had to go back for the whole evening of gaming, about 3 hours-4 hours.. I wasn't doing any hard saves but a whole bunch of f5's (quick saves) instead. BAD IDEA!! I recommend with any and every milestone, HARD SAVE AND QUICK SAVE F5 both just in case your quick saves only take you back so far.. It just takes a minute and there's no worry of your pride. Those who pride themselves haven't had enough experiences with glitches I guess.. I find what seems to be any and all glitches in my games. I don't stay on course.. I venture out if I can. Getting stuck, falling through worlds, blowing myself up, blowing something else up, getting objects to get stuck in other objects and they start a jiggle frenzy of noises trying to get unstuck. lol (Even in PREY, a lot) So if you play like I do and try nearly everything you can in a game, save save save save save <3

the only way to avoid these bug is to save before accept the quest from the cook and dont have the quest to kill him in the log.
in a second and third playtrough over the years i head straight to the cook and the quest works fine.
