Did I mention a ♥♥♥♥ ton of potions? Maybe more than a ton.
Since you didn't bring a ton, you're in for a fun fight. That's the best tip I can give you. Always carry a ton of potions.
With male Rogue, Alistair, Leliana and Wynne: Setup Alistair to beat Sloth, Wynne to heal him and Leliana to "ranged" mode. My own character shoots arrows out of reach in first form, uses cold spell from Spirit form against suitable forms of Sloth - or Burning man form with melee attacks against demon form - and then both Warden and tank go in melee, when Sloth tries to summon blizzard in his final form. Wynne's Stonefist may also interupt the spell, but it usually requires at least two characters beating Sloth in melee combat, or using more powerful spell.
What ever group you have, similar method works quite well. Just make sure that Sloth is only damaging your tank, or you may exhaust your lyrium potions and veins for healing spells, before the battle is over. If he goes against your archer (if you have one), then let her/him run around in circles, while your tank and main character are getting it's attention.
Well congrats, dude, you just killed your only healer.
