Personally i add these two, Skip the fade, once was more than enough. Remove helms because i prefer the look.

Other Stuff:
White Teeth(replaces the generic yellowed teeth textures.)

The following three mods are made by the same author:
Farewell Duncan (adds a small scene between Alistair and Duncan before the Ostegar Battle)
https://www.nexusmods.com/dragonage/mods/4449
Farewell Ser Gilmore (adds extra dialog options to give Ser Gilmore a proper send off; a handshake, a hug, or a kiss)
https://www.nexusmods.com/dragonage/mods/4335
A Warden's Sacrifice (adds a small scene at the end of the game with your love interest if you sacrifice yourself. Or if you have a romance with Alistair and he dies. )
https://www.nexusmods.com/dragonage/mods/4577
The Rescue at Ishal (adds a couple pre-rendered cutscenes showing the aftermath of Ostagar and Flemeth rescuing the PC and Alistair)
https://www.nexusmods.com/dragonage/mods/4690
Companion Mods:
Ser Gilmore Companion (Ser Gilmore survives and can be recruited in Lothering. Compatible with the Farewell Ser Gilmore mod above. Don't install the cc_core_200 dazip file included in the download, it's not needed.)
https://www.nexusmods.com/dragonage/mods/851
Extra Dog Slot (adds a whistle to your inventory that allows to summon/dismiss your dog at anytime, effectively allows you to have a party of 5. He'll be fully functional in cutscenes and whatnot. I only start using the whistle after reaching Lothering and not before getting the dog so idk what would happen. I assume it has protections built in to prevent you from mucking up your game though as I know it gives you a msg saying 'Can't use it at this time' if you use the whistle in the Fade during the Circle Tower quest.)

A Maybe Recommendation...
ZevranASAP (Recruit Zevran early. People in the comments of this mod say they have issues with it, so maybe skip this one or download with caution. AFAIK, it works fine as long as you only use the script version and don't install it before making a New Game. Install it before lighting the Beacon of Ishal. Travel right from Flemeth's Hut to Lothering. Run through Lothering, save Bodan from the darkspawn and travel to Denerim. (Zevran ambush will occur on next travel after first archdemon dream.) After recruiting him, return to camp. Save and exit and remove the script from your override folder. Return to Lothering and finish up quests there normally.

You can also install it using Wabbajack, which is much easier.
Any of the above mention taking care of this?
Lots of fights feel unbalanced.. like a dungeon will be a breeze and WOMP WOMP unkillable boss or something due to some grabbing BS or some death magic BS that are all seemingly unbreakable or unblockable.
Not sure about tree grabs specifically but other grabs like ogre grabs can be countered with skills like pommel strike, dirty fighting, mind blast, shield pummel, etc. I always setup tactics on my warriors to use pommel strike or shield pummel whenever a near-by enemy grabs someone and place it higher up in priority.
(i.e., Enemy > is Grabbing > Activate Pommel Strike)
But I've never looked for mods that re-balanced enemy encounters so I can't speak to that.
