In the wiki you can find all the console commands to unlock the bugged entries for both Origins and Awakening. You can find them in the specific pages of those entries.
But what if you missed some other entries along the way that are not bugged? Some of them are meant to be unlocked only by specific races or classes. The wiki does not list the console commands for ALL the entries...
So, it took me quite some time of research but here's how to do that.

Once you have installed it using the instructions, you can start and load your game.
Open your diary, go to codex and you will find all the entries even if the game does not recognize them as "unlocked". You cannot read them in this form (or at least not all of them) but the titles will be replaced by the console commands that correspond to the specific entries. You can now unlock them in the same way you unlocked the bugged ones. All you have to do is just remove the .plo that is the final part of the command.
For example:
The codex entry for Shale will be titled cod_cha_shale.plo
To unlock it using the console you will type runscript zz_getsetplotflag set cod_cha_shale 0 1
Once you have unlocked all the entries you're interested in, just uninstall the mod and you'll be able to visualize and read them normally.
I tested this method and now I have 100% codex both in Origins and Awakening. The visualization of the codex appears to be just bugged in the DLCs but you SHOULD still be able to use the mod in the DLCs as well, if you need to... (I didn't verify the DLCs because I didn't need them to be complete).

Using the zz_getsetplotflag command on that codex entry will only give you the Dalish Elf version.
