I can play on Nightmare without any issue, i.e. no dying or injuries, using a rogue and complementary companions with decent set of equipment, thoughtful skills/talents, and carefully planned tactics & behaviors. Oh, and, plenty of pause and tactical overhead view to assess current situation and adjust each character's action for high effectiveness, maximum damage, and minimum errors.
In regard to op buiild, resposting my advice/recommendation from another thread:
A dual-dagger, Cunning-based, Assassin+Bard Rogue will produce high DPS, due to fast attack speed with auto-crit and/or auto-backstab, with Dirty Fighting, Coup De Grace, Lethality, Momentum, & Song of Courage.
Pump attribute points into CUN, but enough Dexterity (up to 24, then, 30 or 36) to avoid getting hit and increasing own Attack chance, some Strength to wear leather or dalish armor, and decent Willpower to sustain Song of Courage & Momentum.
Or, Bard+Ranger and use pet summon as a tank, while you shoot deadly auto-crit arrows. Bring along Shale with her sustained Rock Mastery aura for fast missile buff.

Don't bother getting Combat Tactics; spent the skill point on Combat Training and other needed ones. You get enough tactic slots via leveling. Plus, too many slots will just mess up priorities and/or waste stamina/mana on multiple talents on same target.
Lastly, if you set Aggressive or Ranged Behavior, don't use a tactic slot by setting Enemy:Nearest Visible → Attack. It's essentially the same.
Anyway, year-old posts, but still valid. Let me know if you have any specific questions...
