Report a translation problem
It seems like whenever someone is having issues such as this, it's almost invariably because they have VSync off, which messes with the game engine.
I had no idea what triggered this and I ran into it myself after buying and playing in Steam.
After many checkpoint reloads and a few game restarts, I started flailling at the keyboard and found that I could fire in the Tormentor sequence by hitting my Melee key (which I had rebinded to "Q"). After completing the Tormentor sequence I found my mouse button fire controls restored.
I hope this helps for other gamers running into this issue.
After sitting through that head-eating cinematic 50 times, I managed to fix it by quickly pressing the caps lock key twice (which is the default key for changing aiming mode in a toggle fashion instead of a continuous keypress), then I could alt-fire to rip and tear just fine.
Still, this seems tedious. Unlike in the 1st game where the only problem is turning vsync on via the GPU to go through the door, this one seems much more annoying.
Never tried that before, I'll test it sometime to see if that'll also work for me.
