From Nvidia Control Panel?
Max Frame Rate set to 144 (my monitor refresh rate) and vsync off in game, seems to have no effect, the image is still "jittery"...
Nor Nvidia Control Panel or Gforce Experince / App Nvidia are able to interact with the Titan Quest executable, I'm testing this by having 144fps (with Vsync off) inside the game and I'm trying to cap it at 40 using those - noone of them has effect xD
On the other hand, turning off V-Sync from inside the game raises the FPS from 60 to the refresh rate of your monitor, therefore I suspect your override from control panel is doing nothing as well - you got 120 FPS simply because you've turned off Vsync inside the game.
And I also suspect you're experiencing the "jittery image" I'm talking about all along, and you're just not noticing it ^_^'
Its very subtle after all.
Try this, walk back and forth some 2-3 steps in a place with Vsync ON and then OFF (from game options) and keep switching between the two - you should notice the difference
I honestly find that "jittery image" way worse than the input lag, and so VSync has to be kept on
