Another thing to note, if you don't have the expansion you will still have DLC exclusive items dropping / being sold in store you just can't pick them up or buy them.
Atlantis is more of wait for sale kind of thing. It adds what is basically an optional side leveling area outside Rhodes and is basically an act 4.5. The story is fairly short and the quests outside of one only reward experience. Some of the areas are beautiful but seems to have terrible optimization and is prone to slowdowns. Also heard that there's supposed to be bugs with loot causing new gear not to drop (I've only done a single run through the act through so don't have much experience with this point).
Atlantis doesn't add a new mastery like Ragnarok, rather it extends all Masteries from lvl 32 to lvl 40 and adds 2 new level 40 skills to each mastery. The extra mastery points = more stats for your character (and the cost of points spent in talents). The new skills are mostly good and some can make big differences for certain skills and builds.
Atlantis also has a gambler as a gold sink. You buy orbs which drop random loot. Finally, there is a kind of arena mode where you do multiple waves I believe to get better loot (I've mostly just farmed bosses instead through)
Both expand the game's basic system (new weapon type + mastery in Rangnar#k, Rank 40 skills and LvlCap with Atlantis). Both offer quite the scope of new goodies, from sweet new Monster Infrequents to powerful Legendaries, and let's not forget Atlantis' Tinkerer gear....
My advice: When they're on sale, grab both.
I kid you not, but my level 80 hero can not defeat Tartarus in normal level. In fact he gets 1 hit all the time with all resistances maxed except for physical. I consider that part of the game currently broken. He also can't defeat Tartarus in Legendary. The auto leveling seems to work ok until perhaps level 73ish. After perhaps lvl 72, your character really can't add any more damage or resistance or whatever, but it seems that the AI Tartarus still does, so he gets OP at some point. There is a best set up to face him for sure, but I have gotten tired of the multiple equipment/armor outfitting of the late game.
In the quest area, there are some new +3 skill drops fro warrior, spirit, and dream. Maybe more. I have played the 1st 3 areas. Not really into it.
The new skills IMO are either way over powered or under powered for the points needed. Spell bind in earth is too powerful imo. The Spirit top tier spells now make the game laughably easy except vs skels and the broken Tartarus battle. So Atlantis is a mixed bag.
The arena really bugs me. I don't think it has the feel of Titan Quest. The cost wasn't a concern to me.

I don't know about Atlantis because i haven't bought it yet.... but based on steam reviews,it's either wait for sale or no buy....
Buy Atlantis only if you care to own the full Titan Quest experience. I feel it is overpriced.
