How Many Extra Skill Levels From Items?
I've been using the Titan Quest Calculator to plan classes. I take into account of extra skill points from equipment. You can up to four extra skill levels on all skills past the maximum value. Icefrzzy has on their channel builds that have more than four skill points from equipment, some have only four. To me, only four isn't enough as then you need more skill points to reach the maximum level for skills. So I plan to have at least five or six extra skill points from equipment Having five or six extra skill points allows me to free up skill points and get other important skills.

Some of the best and most commly used equipment to increase all skill levels that I've seen on Icefrzzy's channel include:

-Cuttlebone Standard
-Stonebinder's Cuffs
-Demonskin Walkers
-Glory of Belenus
-Brisengamen
-Calypso's Cover
-Corselet of Frey
-Stetheno's Wisdom

Certain artifacts and charms can also increase skill levels. Battle Standard also increases all skill levels by one.

How many extra skill levels from equipment and skills do you you on your characters? What are some other good pieces of equipment for increasing skill levels?

+4 all skill is usually the min goal most builds aim for as this lets you hit skills to max ultimate levels. More +skill means you can reach skill's ultimate skills with less skill investment which lets you spread points around more. Heavy +skill basically turns every skill into one point wonders.

Some builds get a lot of benefit from lots of extra +skill (Soothsayer summoner, for example, more +skill means you can more overall points into all your summon skills + support skills). Others don't use as many skills in their two masteries and are fine with just +4 for ultimate levels.

Stonebinder's Cuffs are the GOAT for +skill, practically every build uses them for that crazy +3 skill in Legendary.

Stheno's Wisdom from your list through does not have +skill. It's useful for some caster builds with +physical resistance and casting speed but not built in +skill.

Some of Icefrzzy's videos feature builds using Stheno's Wisdom and sometimes it does have an increase to skill levels.

Then it'll be from a fix (ie: Hallowed) or +2 skill Crystal of Ereberus socketed, the MI itself does not have +skill from base stats.

Other than a Crystal of Erebus, what other relics increase skill levels? What are some other good pieces of equipment worth using for the skill points?

You can search for 'all skills' on the Titan Quest Item Db in each section for a list of items that give bonuses to skills. Quite a few of them, too many to really list. A number of Legendary MIs give +3 to one mastery as well so depending on your build and the fix they drop in they might be useful.

No gear comes close to Stonebinder's Cuffs, gives almost all the +skill you need by itself and can be target farmed. Drop is rare but not insanely so. The Revenant MI pieces give +2 all skill in Legendary so also good but the drop rate is practically nonexistent.

I've seen on Icefrzzy's channel that some of their petmaster builds use the Glory of Belenus shield if they are not using Stonebinder's Cuffs. They also tend to use the Cuttlebone Standard on caster characters that don't have left click skills using the staff, such as Ternion Attack. I would think that investing in strength to use an item for the skill bonuses would be a waste if you're an elemental character or relying primarily on minions to deal damage.

Is there a limit on how many extra skill points from equipment you can get? I figure that if you have enough, you can get the four ultimate levels on many skills in both masteries.

There is not to my knowledge, however you don't want to go all in on skills just because you can because neglecting other things like resistances can screw you over royally in Legendary as well.
