Most likely an issue with your hardware or perhaps a driver. When was the last time you did maintenance like cleaning out the fans? Could help.

the issue never popped up with other games. it happens with Titan Quest and only with Titan Quest as far as I can tell.

check RTSS overlay and check what's going on? monitor utilisation, temp and fan rpm

it might be because there's no Exclusive fullscreen mode, and you have a 4k monitor/uncapped FPS, that's what games will set it to default as with Borderless windowed. You can try the DX9 mode in that case. I would try investigate

then u gotta check the RTSS overlay, because when I do, the game barely uses my GPU, my CPU does max out at 90%+ though. try the DX9 version

will try the overlay. btw does Titan Quest Anniversary Edition use multiple cores or does it run on a single core?

its single threaded, some things get offloaded to the 2nd core/thread, w/e. You'll see most of the CPU utilisation on CPU0 on RTTS

Is Vsync on only in game or in your graphics card settings as well? Make your system is not trying to run the game at 300 FPS.
