I've had this happen a couple of times. While getting double the fun of two chests is great, it's pretty clear it's a bug by how they've been positioned.

It seems to happen on occasion if I go to repeat a boss I killed prior (this is normal mode). I've seen it both with Nessus and Polyphemus already. First time kills only have had one chest, and sometimes coming back for a second round doesn't always have a second chest there.

I never saw this playing either original TQ or Immortal Throne, only on this new-fangled anniversery edition.

Yeah, it's always been there(since original TQ:IT) at randome when you repeat a boss kill. Unfortunately, it usually means a sub-par drop as well.
