Having this issue and i cant find a fix ahhahaha
I have a 4k monitor but can't stand playing the game at native 4k. I play the game at 1440p (2560x1440) and use AMD Radeon Super Resolution to upscale to 4k, and it looks pretty good. If you have an Nvidia GPU, I think Nvidia has a similar feature you can enable in the drivers.
If you're having issues with the default resolution and/or changing the settings, you can find the settings in Users\<user_name>\Documents\My Games\Titan Quest - Immortal Throne\Settings\options.txt. Just open that file with a text editior and change the resolution entry.
Also, if you still find the game cursor tiny, you can make it larger by changing the Windows mouse pointer size. (Search for "mouse pointer size" in Windows search). The game uses the Windows mouse pointer scaling, so you can make the game cursor as large, or small, as you want by adjusting the Windows mouse pointer size.
I just went to display and used the Large UI setting and its fine at 4K
