You receive it if you talk to Iairos in the temple, or when leaving temple.
Talk to Leonidas south of the Portal, Phaedrus near the northwestern gate and Ariston on the east side of the city center. As soon as you have talked to all three of them, the quest completes.
When you now talk to Ariston you will receive the first main quest of Act V, Troubles of a New Age.
I would suggest verifying game files and fighting Hades again. Use the portal and talk to the priest before exiting the temple. This gives you 2 chances to trigger it and in theory ensures you don't have a corrupted game file.
If that doesn't work you likely either have a corrupted save or some strange issue unique to you. I can confirm that this quest is working. I started act 5 a few days ago on my latest character in normal and it worked fine a couple of weeks back when I went through on Legendary.
