Only four Crystal of Erebus?
So I just completed the quest on normal diff. and double checked all the places, still I only have four Crystal of Erebus in my inventory?

You must have missed one. There are 5 total you can find per diff. 2 are just before Hades, one is behind the warden boss that drops the cell key, one is in middle of a big courtyard and one in a side room which I think is pretty close to the start.
