there is a button in the keybinding options, I think K or L (in default) that allows you to "teleport" (wiggle) a bit from your current position. that feature was originally developed for cases like you encountered
as for issue itself - yes, I had the same problem too once-or-twice and I agree it quite possible happens when you put a teleport over some props, like weapons or armors

As for the issue itself, you often don't have exact control over where the portal appears... and it seems to "like" putting itself on top of things, at least in my case. I've had this happen twice in my latest play session of an hour or so, so not extremely rare.
yes those pesky portals are tricky. once, I managed to put that portal in a "air space near the bridge above the water" (don't know how to describe it better).
