The other thing is creatures can have slow resistance which will shorten the slow duration. The extreme for this example is end bosses. You can't slow them at all. At least my toons never have because they must have massive slow/stun resistance.
Anyway you are correct about the "too little" amount of slow to be effective in higher difficulty since monsters gain boost to theirs total speed(+5% for epic ; +10% for legendary if i recall correctly) and resistances.
anything up to 40% of slow is only visible to the eye in normal difficulty and once you start epic it´s like it´s no longer there. Any amount above 50% in legendary becomes necessary.
I do not know if the skill "heart of frost" is indeed bugged or not, but the debuff is applied by retaliation - meaning it´s only against melee attackers. -> like mentioned above.
