The same way as the Eye of Chaos Quest does. I killed the boss, several times, and the quest still showed as uinfinished. It took multiple exits and relaunches, and running all steps in the Quest to finally have all steps show a check mark and get the quest rewards.
I will send a ZIP file of what I have over.
I'll set it in a dropbox and send the link when I am done.

And I see why not many people actually bother to send log files or savedata to report bugs. This was far more time consuming and complicated than it needed to be. :/
the Eye of Chaos quest was finally completed. On the 6th run.
Had to unisntall and reinstall TQ:AE. Had to re-add it to my AV exceptions list. Opted back into the [publicbeta} branch. Had to reset all settings for the game.
Portaled to Fayum from Memphis. Again. Slughtered jackal beastmen as I ran to the Temple. Danced with scorpions on the path to the Temple, Again. All good fun, can;t say it wasn't enjoyable (except for the unistall, reinstall and re-adding to AV exceptions).
Hiked my little Corsair Tank up the walk to the Temple. Ran around and clicked every Coffer, Large Coffer, Majestic Chest and the Eye Of Chaos altar, while some big Scorion Boss beat on my back while Adrenaline kept me from losing much HP. Sat there and face-tanked the Boss and let him drop my HP by about 15% of total, watched him die, then clicked the Altar. And this time, finally, the Eye DROPPED.
Glad that is over with and I can get on with my game. Frustrating, I love the game, but I don't love spending 2 days to complete 1 EASY quest.
Great that it worked but we are still trying to figure that what's really causing this bug to appear.
When you couldn't finish this quest, the eye while dropping was falling under the ground ot it didn't spawn at all?
Possible that my game files were corrupted prior to the uninstall and reinstall, even though verifying files through Steam was always succesful, no missing or corrupted files found, prior to the uninstall I did as a last ditch effort to fix the issue. Makes no sense to me either.
The only other thought I have is that I started the character on the older *stable* build and opted into the publicbeta after her creation, and continued to play that character.
I'll start a new character in the Beta and run the game again, though I will likely do a different build... Corsair is getting whomped by Typhon repeatedly. >.< I'll see if I can get any exact repro steps that reporoduce the issue consistently.
I had to reinstall windows, saved my old TQ data, installed TQAE, C&Ped the saved data back. I had two characters in the Memphis-Gizeh-Fayum area, both of them were unable to finish or accept any quest. Whenever I talked to the NPCs, they repeated their first text over and over again and didn't accept any quest item. Even though I had both items, Imhotep said something like "Great, now bring me the Eye of Chaos."
I deleted both savegames, C&Ped the old savegames and was suddenly at the first rebirth fountain outside of Memphis with all the quests finished.
The eye appeared for me on the first try. (could easily be just Coincidence, but its stupid easy to try)
I've been running with Nvidia Fast Sync on, from the first minute i played the game.
i wanted to test a wacky theory thats too long to type out, so i set my VSync in the Nvidia control panel back to global setting "Use the 3d application setting" (tripple buffering also on global default of off. as in the Nvidia Driver no longer contols the games Vsync or Tripple buffering. (though dont use tripple buffering with faast sync that defeats teh purpose of it)
Now in the game itself i turned on vsync and tripple buffering.
The First thing i thought when i started playing with Vsync and tripple buffering on, was that the game was in slow motion.
Going from 380+fps down to 60, i was noticing that the animations for Mobs dying and loot seemed to be in slow motion, what i quickly realised Was...... that for the very first time i was seeing ANIMATIONS!!
Before at 380+ FPS, loot appeared nearly instantly, and i certainly never saw it fall. all MOBS also instantly fell/appeaared to the ground upon death.
With Vsync and Tripple buffering on, i now see them (Mobs and loot) have a falling animation.
I Think that at very high fps the animation speed of certing things in the game was causing it to not register certain drops.
Speculation aheaad....
It could mean that the FPS of Animations vs Engine Tics plays a role in the game registering wether or not Loot drops on the ground.
Remember just because your game is running at a certaian FPS, doesnt mean everything else in the game does, there is this thing called "tics", though i have not looked into how Titans Quest does things.

Though I don't know what you mean with the Wusao mountain quest. I had it happen that the door back into Chang An didn't open after I've visited the Jade Emperor. But since there is a waypoint there you can easily teleport and then proceed from there.
