Hecatomb Achievement

I'm 100% sure I've died 100 times. I feel like I've died close to 1000 times and yet I still don't have this achievement. Is it bugged?

The Hecatomb is not bugged, is sneaky to get.
You have to die 100 times don't generally but 100 times in a single game.
For accuracy, when you meet for the last time the devil and decide one of the finals, and restart from the beginning, this will invalidate the achievement (more for the reason that it is almost impossible to die 100 times, over a period of 2000 years).
To get it, when you meet the devil for the last time you don't have to finish the game but keep playing.
After seeing the devil for the last time, every once in a while you will see the ghost, he will ask you if you're not tired of the monotony of life and curse, you answer "no" and keep playing until you get the 100 death. (of course should not be done in a unique run, you can easily do it calmly).
