1. Someone else breaks the vase.
2. You move to the area that player is in.
3. The game connects you to this other player.
On their side, which is the host/server for the instance of the area, the vase is broken. On your side, you haven't seen the vase get broken and weren't connected to the area when it happened, so you see it intact, and it prevents you from collecting the item (a client-side action). You can't break it, however, because that requires you to do damage (a server-side action), and since the server says it's already broken, it can't take damage.
None of the pots in the game which contain (or bar the way to) items are in areas which you can't access in offline mode. Either get yourself to an altar and switch to invitation only mode (splitting yourself from the host player), or go back to the main menu and turn the game to offline mode in the menu, then return to where the vase is and the item will still be there, but the vase will now be breakable, because you're the host for the instance so as long as you see it as unbroken, it can be damaged.
