4. Not entirely sure what you mean or why it should matter. Wasn't watching youtube or netflix or running another game.
I use triple monitors, and I have disabled the other two, but the problem persists. I have also verified integrity of game cache before.

No, but I uninstalled it from the drive it was installed, and reinstalled it in the one the OS is installed (C:), and the problem stopped happening.
All is well now, thanks - we can now resume our regular lives.
