The headshot mastery does state that it is always active vs targets out of action (as in 'unable to engage in combat' rather than 'dead'), don't it? same with enemies that are in patrol and haven't discovered you yet.
I do agree that it is an incredibly powerful thing though
Enemies that are in the Patrol/Alert/Stand-by state don't guarantee a headshot, despite what the description of [Headshot] states. The guaranteed headshot is only available for those status effects you described earlier. The description should probably be made to be more accurate...
