If you have the Kingdoms expansion you should also find the americas, british_isles, crusades, and teutonic folders in there.
After you've done that go to Medieval 2 in your games list and go to properties. Press the 'Set launch options' button. In the next window you should add this:
--features.mod=mods/(mod name)
In the (mod name) location you should add the name of the folder you placed in the mod folder. In my install of the Westeros mod the folder is called Westeros so it looks like this:
--features.mod=mods/Westeros
If done correctly, it should launch the Westeros mod instead of regular Medieval 2 if you select Medieval 2. If you want to play another mod or if you want to play Medieval 2 you should remove the command in the launch options or replace the mod name.
I might've skipped some things but this is all I remember from when I installed the mod.
