please explain reputation to me
My reputation with milan randomly went down to poor for no reason. All i did was pass my diplomat through their territory. I wish reputation was explained a little better or atleast told you the reason for the change.

You seem to confuse global reputation with individual faction standing. The latter can change by simply making agreements with enemies of theirs, or overstepping into their regions with armies. In the later game simply being more powerful suffices.
The former requires actions like exterminating a population which has a global impact regardless to which faction the settlement belonged. Ditto with executing prisoners, stuff like that.

well I have not done any of the things you have listed and my relations with them went from poor to very poor. they are just hating to hate i guess

There is a mechanic that slowly deteriorates faction standings every turn. It's minute - but it accumulates, guess that's what happened.

In other words: you need to work on it if you wish to maintain faction standing levels. Just like in real life I guess :)

The odd florin dropped into a diplomats hand (not a bribe, mind you) does wonders here.

What you need is to unzip some files that are in the game, the way you play changes a lot, the AI is more aggressive; There are some guides on Steam so you can do it easily, it's just changing a couple of files

I certainly would like to know which file(s) that would be - every single file that is included in the 'pack' files is getting read and used by the game and afaik there isn't a single ZIP in there.

There is however a bug in the original descr_faction_standing file (that's the one I get all my info from that I referenced here) that will punish you even if you only occupy a settlement.
That bug is fixed my Bare Geomod mod set up (which is the default campaign with a number of fixes) if you are interested (simply play it instead), the alternative would be to unpack the game and make it mod compatible and edit the file yourself.

Encroaching too close to their borders?

Or is Milan just a total jack ass?

the steam version of the game has a bug in the files that affects your standing with other factions whenever you occupy a rebel settlement, because it acts like you are razing it. I fixed it now and now nobody hates me for no reason. Before i would have france spain and portugal invade me almost at the same time, when I have not done anything to provoke such hostilities very early on in like turn 20-something.

Correct - that's the said descr_faction_standings file and bug I referred to earlier.

Same file has the 'tall poppy' trigger that will reduce faction standing when you reach a certain faction ranking. That's on top of the default trigger that reduces faction standing.
And there are obviously also 'enemy of my friends' triggers.

If left unchecked\countered those reductions will keep on accumulating and possibly lead to hostilities without the player making any transgressions.
