Guild HQs are limited to one per guild for the entire world. But there is nothing stopping a faction having multiple HQs for _different_ guilds.
Master guilds (or major houses) are theoretically limited to one per guild per faction, but with the loophole that you can obtain extras by conquest. It's perfectly normal to end up with several Master Thieves Guilds (which doesn't really achieve anything), and not uncommon to get a second Master Swordsmith (which is nice because the experience bonuses stack).
