Not long enough in vanilla to worry about it. It's gone within 10 turns.

Whenever the plague ends, you get the notice saying so, and it has reached that "unfortuneate destinations" whereever that is.

Like 20 turns, unless you keep moving your troops. The trick is to move all agents into the settlements and not move ANY one as long as the rat sign exists.

If you put spies in plagued settlements they can catch it and then spread it to enemy settlements.
