Now I play it for mods that modify the game to be like Vanilla, except enhanced by the modder's ambitions.
Overall, I started the game(s) with its vanilla content and (especially with Kingdoms content combined) and didn't feel like it needed any mods whatsoever. The game is fine just as it is for countless hours of play. However, the mods provide mostly divergence for people who may get bored of the vanilla content, so it is understandable if they buy the game just to play what kind of content they like most (fantasy&LOTR stuff like you mentioned).
For me, I stick with historical mods like Stainless Steel and Broken Crecent for now, as I am basically a Medieval nerd. Despite what I also said in the intro part, I like first Medieval's dark in Risk-style atmosphere more than the possibilities and polishing of the second; but in the end, both are incredibly good games that assure many many hours of gameplay, with or without mods.
Just download some files here, move some files there, copy and rename a file here, and viola!

Though I would recommend you start with mods like Total Vanilla Expanded or Expanded Americas if you don't want to go plunging yourself into mods that vastly change the game or even the world.
