Report a translation problem
In practice I mean, for example, that part when you follow her to Faculty Office 2, and she stops by the door and waits for you to open it. If she says something like "weren't you going to help" or something similar, you took too long to open it and may have screwed it up. But that's easy to deal with, just go straight to the objective and get there before her (of course, do what you have to do, like using the key or whatever, but do it before she asks), this will trigger the cutscene and won't negatively affect the affection. Same for the other "follow" or "be followed" parts, just go straight to the point.
In the beggining I was afraid that making her run would reduce the affection, but apparently it doesn't (I got WC ending 2 times even though I made her run a lot, so don't mind that)
If you're already doing that, I don't really know what you're doing wrong, since the answers seems to be correct... I'll confirm that later when I get back to the game, but I think it's correct.
Another point to note is, to NEVER interact with her at all throughout the playthrough, EXCEPT for the part where you return her diary and to give her the coffee. Just to be save.

But there are 5 circles on WC, which means you will have to beat it on every difficulty to get 100%.
It is indicated by the 5 dots per ending, each representing the difficulties from Very easy, Easy, Normal, Hard and lastly Hell.
I also confirm that this information is wrong, you can get all endings on all difficulties (and you need to do it in order to complete the ending collection).
Maybe you're confusing with the original 2001 game, I remember it having some unattainable endings on lower difficulties.
wait a minute, so you are telling me that if i dont follow step by step this guide i wont get the ending? and even if i do, there is a chance i dont get it anyway because maybe i took 1/2 a second to long?
i was expecting that key conversations decided the ending, not every breath i take
I even tested again waiting a while to open doors, leaving her behind and everything and still got the White Chrysanthemum ending, so maybe that was an error and they fixed in a patch or something? I know it' been way easier to get the ending later after launch...
I'm not 100% sure how it works since it's not explicitly clarified to you in the game, but it does have some kind of affection meter for each character, so the routes you take + the choices you make (or at least some key choices, since some appear to only change dialogue and not the ending) are the main things to affect your ending.
Some minor things like being a pervert and well, "interacting" with the girls skirts also lower your affection meter, and I THINK there's something for dying too much but I might be mistaken for some other game, since I didn't play this one in a while.
Anyway, main things are choices and routes.
