Pandemonium Token
When you use the Pandemonium tokens, sometimes an outcome is just "ACTIVATED", what the hell does it do?

The same thing happens in the nether realm bar on one of the barrels.

It looks like a reference to the Diablo 2 Chat Gem, which you can google. The gem in question did nothing but produce a number of meaningless and confusing messages.
