Psychotherapy Achievement?

Any ideas how to unlock it? If you look at the achievements, you can see, that there are 2 different pictures, one with the therapist and the other one with a locked TV. Maybe we need to input a code at the level select?

After the Electrohead mission you can talk to your therapist and refuse the drug if you pick the right dialog options. It seems to be the only thing you're missing to get the achievement.

Yes, this must be it, thats what was missing for me.
DON'T SPAM the red interrupt that in the therapy session after Electrohead.
A lot of them say something like "I want my medicine"
DONT PICK THAT ONE
Let the therapist talk until you can refuse to take the drug.
You have to do so multiple times.

I did a full playthrough again and managed to get the achievement after:


Hanging up multiple times in the first mission

Having the receptionist killed

Hanging up before radiohead, talking to radiohead and refusing the medicine

Killing all the guards in the prison level

Saying "♥♥♥♥ You" to the therapist in one of the dialogues (most of these sequences I did only the interrupt messages).

Not getting spotted by the camera guy in the slaughterhouse and skipping the tapes (you get a message about that you should have gotten more information after that so it might be required for the achievement)

After that I got the different event during bunker 2 that triggers the achievement.

I got the achievement and holy actual f*ck... Like I am just kinda stunned. But yeah this is definitely a favourite game, no question. I have only two things to say since I know that the devs were on the threads. One I really, really love the headshots in this game... like... it is just so visceral... so I think it would be really cool if you make something that uses guns a little more <Doesn't have to be the same series or canon but like... you clearly are good at making gunshots satisfying> and since I heard you were making more content for this game and then a second game to follow, I have a few questions regarding that... maybe they're questions I dunno.

Would we get to see more of the null agents throughout, like a Katana Fifteen Game... for example, and will this game's story be tied up in the content patches? Now I don't mind paying money for more of these games, however I would rather have the whole plotline for this game in this game, and a separate game be separate, <So instead of two parts released as two games this game is wrapped up at some point and a second game following another story arc or character is released...> Again it is more convenience then cost because I do enjoy this game. Sorry that this is a little random but I figured it was worth putting out there.

The most annoying achievement in this entire game. Just frustrated after 2 attempts at doing this starting from the beginning to end and I guess I'm missing something here. Maybe it was when I accidentally argued with shrink to the point where he gave me the medicine since I didn't choose the option to say no to it, and then I reloaded the level again to change it to the correct choice and continue from there? Not even sure but holy hell is this one the most annoying one for simply requiring things to be so specific with their timed dialogue.

You dont need to replay the whole game just the few missions where you can beat them but go against orders so the club where you can talk to electrohead and take the drugs, The prison where you can kill everyone. then just keep interrupting him and telling him to shut the ♥♥♥♥ up skipping the therapy and going back to menu when the session is over

then just go play bunkers therapy then go to bunker 2 and get ready for some payoff

If I don't have to do it from the beginning then I must have a bug, because I've followed all steps from everyone and I keep getting the same ♥♥♥♥ each time.

EDIT: NVM it seems as if the first few missions I had to do certain dialogue choices instead of skipping or looking for insults.

I have done all of this except maybesaying "♥♥♥♥ you" but i don't think i got the chance anywhere, didn't get the cutscene.
EDIT: is it a V hallucination where you have to say "♥♥♥♥ you"?

I think those are not necessary for the achievement, you don't say ♥♥♥♥ you to the "real" one (it's just a chronos hallucination that turns into V) & in my playthrough I ran into Leon's sensors because I was afraid the message about not learning about NULL might change the outcome somehow. Still got the achievement.

Might require further testing/a bigger sample size for the first one though

Mine just popped up after I repeated the tutorial level, hung up the phone so the guy went angry and finished the level.
Then, just in case, i went through the Hotel level, annoyed the receptionist, let the target jump to his death, killed the guards at the end and finished the level.
So I went through the club level again and did the Electrohead thing and refused the drug again.

That's it. I think the only thing that was missing was the tutorial level section

My advice to anybody would be to replay each level specifically where you see the therapist. In those sections look for dialogue that sounds insulting to him, and choose those. If you don't find any, then simply replay the ones that didn't have insulting dialogue and those can be used to simply interrupt him/skip dialogue (except for the one after you TALK to the DJ about the drugs which may be the beginning of the prison level tape, in which you actually talk to him normally and keep saying NO to the drug).

Then you follow what everyone else has said here and you should be fine. It took me forever to figure it out because I ignored the first 2 levels/meetings with the therapist which I think I had to keep interrupting the therapist and demanding my drugs at first before talking to the DJ. Me interrupting him to demand the drugs was also something that angers him and helps secure the "I'm sad you're still alive" activation you're looking for that he says in the beginning of Bunker 1.

After trying to trigger a bunch of flags people from all over the steam forums as well as on youtube have suggested, i finally got the cheevo.

Believe it or not, i had all of the flags triggered except for one. During the first mission, after you hang up on your contact a bunch of times, you eventually either have to say "sorry about that" or "hello" to be able to continue playing.

Turns out, apologizing stops the achievement from being obtainable lol. So if you tried seemingly everything and still don't have it, maybe that's what you're missing.
