A quick way to check this theory is to unplug the mouse while playing.
Those icons were listed in the Controller Settings under something called vJoy Device, which the game kept defaulting to for some reason. I looked it up and it's some kind of driver that lets you play with controllers not natively supported by PC I think. I guess it came preinstalled on my computer? I don't think I need it since the Xbox 360 Controller is natively supported by Windows, so I uninstalled vJoy Device and sure enough, God Eater's controls started working! No more camera aiming up or button icons switching, so it was vJoy Device that was acting up. The mouse icon still flickers for some reason, but it functions correctly.
Not sure why only God Eater of all my games was having issues with vJoy Device, but hopefully I didn't need it for anything.
I guess it's just the way the game scans for input devices.
