PS4 Controller not working properly, Vanquish - Scarlet.Crush Productions

Connected PS4 controller via usb to my pc, started Vanquish, several buttons are wonky (e.g. A | B, X | Y swapped, start button does not work, R1 button sticks, etc.). This issue is only happening with Vanquish, controller works fine with EVERY OTHER game in my library; which means I'm not re-installing, rebooting, trying other software. Any other practical fixes on the software side, I'm open to. Thanks.
