Where do you found it? It must be somewhere in the Deadwatch but I already cleared it 6 times on legendary without finding it. A screenshot would be nice to know how it looks like.

Did you do the stuff in the graveyard west of deadwatch? Not sure if it was there but I remember I cleared deadwatch once, did the stuff in the graveyard and then went to caldeus

It comes from a skull pile resource in Deadwatch. The difficulty doesn't matter. I'd run it on the easiest one and search for skull resources in each room. Getting the surveyor should help finding out which room has resources in it.

I'm having trouble with this one too. I've searched Deadwatch top to bottom but absolutely cannot find it. What am I missing?

Edit: never mind, I reset the dungeon and it appeared, so I'm guessing it doesn't turn up every time?

Is that in the graveyard west of deadwatch or in deadwatch. I have done deathwatch three times and I haven't found anything.

That skull pile in the picture. Has people found it always spawns there or no? If so could someone just screen shot the dungeon map screen to show which room it spawns in, please? Would be very helpful! :)

Its completely random. Sometimes there is a skull pile, sometimes not.

It took me around five runs to find the skull. It definitely spawns only in the 'skull mound / pile' as shown in the picture in this thread.

I really think this is a major flaw in the game having key items or events for those items be randomly placed in a dungeon.

I've done Deadwatch three times now. Once on normal (no skull), once on heroic (no skull) and once on legendary (got the skull there). The location was not the same as someone posted a photo of, but it did indeed drop from one of those "bone piles" that you loot, as can be seen in the photo.

I think the RNG on this one is a bad design choice, only because the reward is a weapon that is only going to be useful if you haven't run that dungeon a dozen times to get the damn skull and gained way too much exp in the process. My first playthrough I got the skull on my first run, on my NG+ game I did it about 6 times and never got it, not even remotely worth trying to get now.
