I'm having the same issue with my Xbox One controller, but my 360 controller works fine.
I think it has to do with the Windows 10 anniversary update. Microsoft ♥♥♥♥♥♥ up the xbox one controller drivers and it is showing up as both a xinput and dinput controller, so that's why the buttons are all goofy. There is a fix for it though.
Dragon's Dogma was having this same problem, but Capcom was nice enough to post a little fix to get it all sorted out.

A little tidbit though, I followed the directions exactly, but then my controller wasn't recognized at all. What worked for me was to just have the driver update search automatically for an update online after going through the instructions from the post. After that it works fine and the controller behaves like it should.
Hopefully this fixes anyone's problem.
1.Win key + X then click on Device Manager
2. Look for Xbox peripherals then click down arrow
3. double click Xbox Gaming Device
4. click on driver
5 click on update driver
6. make sure driver date is 10/29/2015
It not game fault it your fault not update your drivers
Imma have to try the fix.
then you stay on console then cant figure out how update your driver
follow these steps, it fixed and works perfectly now. Wolf posted this link earlier in the thread.
