2) That should be good for a heart piece, not just decoration.
3) There are plenty of breakable items in the Wastelands that don't hide anything, so it's possible they're just decorative elements.

You should have 20 hearts and nothing in the "Expansions" slots. Obviously, I'm going on my findings on the Steam version and I'm still hoping any Switch players can validate that my guide is accurate for their version as well.

This is exactly how my inventory looks like, thank you! I can't tell you if your guide also works for the switch version, since I used it only for finding the "first" statue in Golem's haven (I also don't remember to have find a heart by exploding a wall in the frozen temple, but I am quite sure the problem comes from my memories).
The Wasteland ones indeed looked as decorative elements, but the hole at the right riverbank of D5 sounded suspicious to me! I can't find (or find again if I already visited it...) a way to reach it, do you remember how you did it?
I also see that you get ten butterflies and ten flowers: did you try one of each in purpose? I kept all the ones I found by digging, buying, etc. and I have 11 of each.
Finally: do you think the gold is capped? I decided to farm orc skulls to check that, but get bored at 5555 gold... Same for the soldier who counts the number of monsters you killed, I am near to 7500 and hesitate to push untill 9999/10000.

I've considered adding screenshots for the collectibles in dungeons, but I haven't yet since it would require a massive overhaul in the structure of the guide. Unfortunately I think the only way I can suggest to check that would be to warp back to the dungeon and check.
I suppose if anyone would want to validate the guide against Switch, I'd probably recommend starting a new playthrough and checking off each item as they find it, which is pretty much how I produced the guide in the first place.
The simplest way is probably to use the Runestone Temple warp then go due east. When you're in C5 go up the stairs near the top of the area to continue east. You should be able to see the hole when you enter the D5 area.
That's just by chance, and all of the random drop items and consumables are going to have different counts on different playthroughs.
Good question. I haven't challenged these so I don't know offhand. Gold is fairly easy to run up in Snowy Summit, so I could try that. I checked in with the Knight Commander and I'm only at 7254, so I'd have to run around a while beating stuff up (or try editing my save file).
Update: I made some changes to a copy of my save file and set the gold and kill counts to 9998. I went out and hacked up a few mushrooms and found that both can go above 10000. What I found was... strange? The kill count doesn't seem to have a practical limit, so an educated guess says it might go up to 2147483647 but then who knows what happens after that. Gold was odd though. It hit 10000 and kept going but when I exited the first time it reset it down to 10000. I reloaded and when I picked up a few more coins and exited, it saved it without resetting. This might not make a difference with the Switch version, but it was interesting to see what would happen.

How do you run up gold in Snowy Summit? If you haven't the time to check these points and want to have an answer, I can try it for you tonight (well, I would begin by the gold).

Oh! I know where you mean now. That hole is an exit for another cave, but I can't recall which one offhand. I'm pretty sure the entrance is somewhere in South Blossom to the east of this screen.
Find the archaeologist. He'll ask you to guard him while he searches for treasure and if you're successful you earn a large amount of gold. There doesn't seem to be any limit to the number of times you can attempt it, so it's easy to farm gold there. For my purposes it wasn't necessary though. See the update note in my previous comment for more info.
