You then send your diplomats off to turn them to join you. Depending on that diplos stats, it can be anything from 8-100 days(if they really suck!) so stick to using the key people who have that skill.
Sometimes when one planet turns, others will join you also, or popularity will increase on other planets.
Outer Rim planets that dont have any facilities on them (like a mine) or any kind of population will just be an empty planet, thats where you drop a troop unit from a ship that MUST be in orbit above. Once its yours, you can move troops around freely without ships.
