How to change key bindings?

I want to change the middle mouse button to something else. How do I do this?

Additionally, it's annoying when I start the game with KBM and want to switch to controller or vice-versa and have to literally quit the entire game to do this, even if there's only one player.

In the main menu there's a cogwheel. You can change the bindings there. The option you're looking for is "Cast Self".

You don't have to quit the game. Press ESC once to remove your keyboard character. Then press A or whatever the join button is on your controller to join the lobby with the controller. The character slots have two different icons, one for KBM and one for controller.

While I'm at it; is there any way to move the character via WSAD instead of holding down the mouse button? And to map the spell keys to the number keys or something similar?
