Report a translation problem
The first you can miss is Nosiest Neighbor, since it's really easy to miss a handful of the bits of character info. Some of them are time-sensitive, and many of them require talking to characters 2-3 times. The only guide I know of for this is in Japanese, here (http://falcom.xfu.jp/wiki/wiki.cgi/zwei2?page=character), but it has a table for all the info divided by character, as well as all the info split up by sections of the game at the bottom.
The other one you can miss is Drawn Into the Spiral, for completing the trading sequence for Ragna's Spiral Gear. During the trading sequence, one of the items you get (Mysterious Pet Food in the original Japanese, haven't gotten that far in the English), you need to trade to Huang for the next item in the sequence. You can, however, feed it to a pet instead. If you feed it to your pet, you'll fail the trading sequence until your next playthrough, so, you know, don't do that. Especially as the Spiral Gear is a really good weapon.
To answer your other question, no, I'm pretty sure it's not possible to get all the achievements in your first playthrough. There are armors that are unavailable until your second playthrough. I think there's also a pet that's second playthrough only, although I'm not 100% on that. There may be some other things that get achievements that are 2nd playthrough only, too, but it's been a while so the armors are the only things I remember with certainty, so Wondergarments and Fundergarments can't be gotten until then.
Hope this helps! For what it's worth, the game has a lot of fun extra content in the 2nd playthrough, and if you get a high hunter rank in your 1st run, you start off with a load of cash in the 2nd, and you get food on your 2nd based on the amount of treasures you got in the 1st. If you turn in all the treasures, you get enough food to kick you up to like level 28-30, if I'm remembering correctly, so it's a nice way to really speed up the second run!
