This would be my guess as well. If the power is near max, then one heat exchanger can only support two turbines, similiar to regular steam engines.
Are you sure you are using the steam turbine and not the regular steam engine?
Steam power can trick you a bit if you are only using a very small percentage of your available capacity. Under small loads, every steam turbine will be cranking out some power, indicated by the steam being released from its vent. However, as demand increases, the first few turbines will use an increasing amount of steam, and some of the turbines later in the chain will shut down until eventually only two-per-heat-exchanger are left running at maximum output.
