You know, on a side note this does give me an idea for a potential Trypanophobia trait. Maybe if I ever get into modding I'll add something like that.
But yeah, I'm also super unfond of needles, and this exposure therapy idea is, imo, bupkiss.
I get the joke that people are wanting to make, but I can also imagine how tiring it is for people with phobias.
But exposure therapy isn't really meant to be done during recreation. You're more relaxed, which makes the emotional difference between being unexposed and exposed larger, which accentuates the fear response.
Instead, it becomes flooding therapy, which is overall less effective, frequently traumatic, and doesn't even work for many people.

worst case, this would be very easy to solve with a mod!
nexus mods already has people running around as hyenas, swapping the needle model for something else would be even simpler.
