The reaper seems to be OP in multiplayer
That thing respawns INSTANTLY in multiplayer coop, and knows exactly where you are at all times. Is it just some local bug, or did something this big and annoying actually pass through quality testing somehow?

Our co-op group didn't seem to have any unusual issues with the Reaper, but that was weeks ago, and we're pretty much done with the Security Sector. It did seem to hunt us, but that felt normal based on my solo play.

Speaking of, that sounds about right for solo play too, since it would respawn somewhat quickly near by. Wasn't uncommon for me to hear it show back up less than a minute later and wandering around near me, but not towards me.

Now if it respawns within a few seconds of being banished and always path finds to you several times in a row, then that might be an issue. Though the last time I tried hunting it down, I would accidentally walk away from it and not see for several minutes at a time.

I don't think there has bee any update since the last time our group was progressing through that sector, so I'm not sure what the issue is.

It basically grabbed me, instantly appeared right next to my friend and kill him, and then appeared right next to me when i got out of the portal world and took a step closer to the surface elevator. It did this ♥♥♥♥ multiple times in a row

Yeah, that's hyper aggressive. Not sure what changed or what the glitch is, but it shouldn't reappear that quickly. Though, it showing up (almost) next to you is possible. I've had that happen a few times. I think that's a result of standing next to its respawn points.

Are you on a dedicated server? Any idea on how to replicate it?

played on LAN with someone (same wifi) on their save, just reached the security sector and started looting everything. It was instantly respawning from the moment we stepped in

Best I can offer is if it happens again next time you play have them send the save file over via the bug report forum. That's really weird.
