That, and there's this relatable feeling of fear knowing something is following / stalking you, and the remedy is to stare at it for roughly 10 seconds (I imagine it might be like telling someone with a fear of heights to look down a tall tower, for some), or run away (assuming you aren't cornered in the office sector, looting rooms, where you may feel helplessness like the teens in those Jason slasher films).
I do know that some people love the Leyak, some others strongly dislike it (I know the Leyak was the reason I took a break from Abiotic Factor for a week... I'm a scaredy cat who didn't expect this kind of "threat", and it did make me feel very uncomfortable. Eventually, you kinda just get used to it (getting the X-Ray tower ASAP helped me feel more comfortable facing it... aside from calling it Greg / Bob / Steve / etc.)). If it does trigger some form of fear response (I'm no expert, just to point that out), taking a break can help, and if you don't mind spoilers, you could look up more information on the Leyak / IS-0091, to kinda build your "resistance" to it.
Though, I'd say the charm of the game is facing the unknown / exploring new horizons, though I can't say I fully understand how this can affect certain players (I want to imagine if there's some form of phobia / trauma that triggers a fear response in some players, there could be an option for it, I mean, there is an "Arachnophobia" toggle in the Accessibility options... I'm still not sure which enemy that applies to prior to adding the "Symphonist" enemy type).
Will just have to wait and see how the devs will address this concern (hopefully it's just a toggle in Accessibility, or option to "store" it in the cell it was in, or just have a trinket that will make you a non-target (gameplay wise, you sacrifice a trinket slot for safety / ease of mind). Hopefully something that keeps the Leyak in-game, but doesn't deter players too much from experiencing this wonderful game.
Personally I fall under the "kind of annoying" camp. It's not a big issue for me but it does get a bit grating having to lug around the xray flashlight or sit in place for a couple seconds staring at it every ~15 minutes.
The only time it's ever a problem is when a co-op partner goes AFK. Then I get to sit around in a state of anger-dread, wondering if I'm going to have to welcome them back with "your stuff is over there".
First few "jumpscares"? Neat.
Then it got tedious and you're left having to stand and stare/ x-ray the thing every so often when you're already losing your mind getting railed by rng trying to get a rare drop from specific enemies/ general resource gathering.....
It's literally the Marauder from DOOM Eternal's distant baby cousin. A distraction at best, an annoyance at worst. Omitting it won't change much, if at all, once it served its purpose.
...
Then the thing got irritated. I would be running around, exploring, ambushing, and looting, and then I'd hear these quiet stifled utterances and sighs. I'd groan in annoyance, drop what I was doing so I could hunt for it and a glare at it for 10-20 seconds, since it was a big liability to ignore it. For me it was the regular interruption of gameplay that got tiresome. Even with the hand held X-Ray Lamps, it was still stop and glare/shine to make it go away, but it was a bit faster and I got a radioactive ball for my troubles.
As for the drama behind it, I never had an issue with hearing it, but other people do, and I can totally see why Leyak would be a pain in the butt for them, since the only real and reliable warning you get is via sound; and that sound could be easy to miss for several reasons.
